Trintellix (vortioxetine) is FDA-approved for the treatment of major depressive disorder (MDD) in adults. It is a multimodal antidepressant that acts on several serotonin receptor subtypes in addition to blocking the serotonin transporter.

While SSRIs work primarily by blocking the serotonin transporter, Trintellix also directly modulates multiple serotonin receptors (5-HT1A, 5-HT1B, 5-HT3, 5-HT7, and others). This multimodal activity may contribute to improvements in cognitive symptoms such as concentration and memory in addition to mood.

Most patients begin to notice improvements in mood within 2 to 4 weeks of starting Trintellix, though some may experience early changes in sleep and energy sooner. Full therapeutic benefit often takes 4 to 8 weeks. A physician will monitor your response and adjust your dose as needed.

The most commonly reported side effects of Trintellix include nausea, diarrhea, dry mouth, constipation, vomiting, dizziness, and sexual dysfunction. Nausea is most common when starting treatment and often improves after the first week or two. Taking the medication with food may help reduce nausea.

The recommended starting dose is typically 10 mg once daily. A physician may increase the dose to 20 mg per day based on your response and tolerability, or lower it to 5 mg per day if side effects are a concern. Dosing decisions are individualized and require a clinical evaluation.

Trintellix should not be taken with monoamine oxidase inhibitors (MAOIs) due to the risk of serotonin syndrome. Caution is also advised with other serotonergic drugs, certain antifungals, rifampin, and strong CYP2D6 inhibitors, which can affect vortioxetine blood levels. Always disclose all medications and supplements to your physician.

You should not stop Trintellix abruptly without speaking with your physician. While vortioxetine has a relatively low risk of discontinuation syndrome compared to some other antidepressants, your doctor will recommend a gradual taper when it is time to stop to minimize any potential withdrawal effects.
