Most patients are treated with antithyroid medication for 12 to 18 months. After this period, a trial of discontinuation may be considered if thyroid hormone levels have normalized and antibody levels have declined. However, some patients have a relapse and require longer treatment, repeat therapy, or a definitive treatment such as radioactive iodine ablation or thyroid surgery.
Antithyroid medications take weeks to lower thyroid hormone levels, but symptoms like rapid heartbeat, palpitations, tremor, and anxiety can be debilitating in the meantime. Propranolol blocks the effects of excess thyroid hormones on the heart and nervous system, providing rapid symptom relief while the antithyroid drug works. It is not a treatment for the underlying thyroid overactivity itself.
A rare but serious side effect of both methimazole and PTU is agranulocytosis, a sudden drop in white blood cells that increases infection risk. Patients should contact a physician immediately if they develop fever, sore throat, or mouth sores while on these medications. PTU carries an additional rare risk of liver toxicity. Regular monitoring of blood counts and liver function may be recommended by your physician.
