It works by slowing the breakdown of bone, helping to maintain or improve bone mineral density and reduce the risk of fractures.

Actonel should be taken in the morning on an empty stomach with a full glass of plain water, at least 30 minutes before eating, drinking anything other than plain water, or taking other medications. After swallowing the tablet, you must remain upright (sitting or standing) for at least 30 minutes. Do not chew or crush the tablet. Follow your prescribing physician's instructions for whether your dose is once weekly or once monthly.

Common side effects include stomach pain or upset, nausea, constipation, diarrhea, joint or muscle pain, and headache. Some people experience esophageal irritation, especially if the tablet is not taken with enough water or if you lie down too soon after taking it. Rare but serious side effects include osteonecrosis of the jaw and atypical femur fractures. Contact your doctor if you experience severe jaw pain, thigh pain, or difficulty swallowing.

Actonel begins reducing bone resorption relatively quickly after you start taking it, but meaningful increases in bone mineral density are typically measured at 6 to 12 months on a bone density scan. Fracture risk reduction can occur within the first year of treatment. Your physician will monitor your response over time and advise on how long you should continue therapy.

Actonel is generally not recommended for patients with severe kidney impairment (creatinine clearance below 30 mL/min) because the drug is primarily excreted by the kidneys and adequate clearance is needed to avoid accumulation. If you have known kidney disease, be sure to share your most recent kidney function test results with your physician during your evaluation.

Your physician may recommend adequate calcium and vitamin D intake while you are on Actonel, as these nutrients are essential for bone health and can affect how bisphosphonates work. However, calcium supplements or antacids should not be taken at the same time as Actonel because they can interfere with absorption. Take any supplements at a different time of day, and follow your doctor's specific guidance.
