Does Hydroxyzine Work Better than Benadryl for Anxiety?

Hydroxyzine has some evidence for short-term relief of generalized anxiety and is prescribed for anxiety in some settings. Benadryl has not been established as an anxiety treatment, and direct comparisons between the two are lacking. Both medications are antihistamines that cross the blood-brain barrier, but hydroxyzine is specifically prescribed for anxiety disorders with typical doses of 25-100mg daily. Hydroxyzine generally remains in the body longer than diphenhydramine, but a longer half-life does not necessarily mean better or more sustained anxiety relief. Both medications can cause drowsiness and impair attention, coordination, or driving; individual responses vary. Short-term trials suggest that hydroxyzine can reduce symptoms in some people with generalized anxiety disorder, but the evidence is limited and response rates vary by study and definition. Benadryl can cause sedation within a short time, but sedation is not the same as treating anxiety, and it should not be used for acute anxiety unless a clinician specifically recommends it. Hydroxyzine can also act relatively quickly, but timing and dosing should follow a prescriber's instructions.
