Diltiazem blocks calcium channels in the heart and the walls of blood vessels. This slows the heart rate, reduces the force of contractions, and relaxes blood vessel walls, which lowers blood pressure and decreases the heart's oxygen demand.

The extended-release formulation of Cardizem is designed to release the medication gradually over the day. Many patients begin to notice improved rate control within a few hours of the first dose, but it may take several days to reach the full therapeutic effect.

Common side effects include dizziness, headache, flushing, swelling of the ankles or feet (edema), slow heart rate (bradycardia), and constipation. Contact a physician if you experience significant swelling, fainting, or a very slow pulse.
