Why Does My Back Hurt When I Breathe Deeply?

Back pain during deep breathing typically indicates musculoskeletal strain or inflammation affecting the chest wall, ribs, or spinal muscles. When you inhale deeply, your ribcage expands and engages intercostal muscles between the ribs, which can trigger pain if these areas are injured or inflamed. Common causes include muscle strain from poor posture, sudden movements, or overuse during exercise. Rib injuries, costochondritis (inflammation of rib cartilage), or thoracic spine issues can also cause breathing-related back pain. Studies show that up to 23% of chest wall pain is musculoskeletal in origin. However, sharp or severe pain during breathing can sometimes indicate more serious conditions like pneumonia, pleurisy, or pulmonary embolism, which require immediate medical attention.
