Antacids containing calcium carbonate (like Tums) and magnesium hydroxide are considered safe heartburn medications during pregnancy. The American College of Obstetricians and Gynecologists recommends these as first-line treatments because they work locally in the stomach without significant absorption into the bloodstream. If antacids don't provide relief, ranitidine (H2 blocker) is generally considered safe after the first trimester. However, avoid sodium bicarbonate antacids as they can cause fluid retention and affect electrolyte balance. Proton pump inhibitors like omeprazole may be prescribed in severe cases, but should only be used under medical supervision.
