Does Tirzepatide Cause Brain Fog?
Medically reviewed by Andre Stone | MD, University of Pittsburgh School of Medicine on August 25th, 2026.
Some people taking tirzepatide may report difficulty concentrating, mental fatigue, or feeling mentally "cloudy." However, brain fog is not a well-established adverse effect of tirzepatide, and clinical studies have not established that it affects 3-5% of users. Symptoms may have several possible causes, including dehydration, reduced food intake, or low blood sugar, particularly when tirzepatide is used with insulin or certain diabetes medicines. There is not enough evidence to predict when these symptoms will begin or how quickly they will improve. Contact your clinician if cognitive symptoms persist or interfere with daily activities, and do not change your dose on your own. Seek prompt medical care for confusion accompanied by symptoms of low blood sugar, and seek emergency care for sudden confusion, trouble speaking, weakness, fainting, or other severe symptoms.
