What Causes Knee Pain Without Injury?
Medically reviewed by Andre Stone | MD, University of Pittsburgh School of Medicine on August 25th, 2026.
Knee pain without an injury commonly stems from overuse, osteoarthritis, or factors affecting how the joint moves. Other possible causes include gout, inflammatory arthritis, and, less commonly, infection. Osteoarthritis affects over 32 million Americans and commonly causes gradual knee pain without specific trauma. Patellofemoral pain syndrome causes pain around or behind the kneecap and may be related to overuse and several factors affecting how the knee and hip move. Bursitis develops when fluid-filled sacs around the knee become inflamed from repetitive motions. Other causes include Baker's cysts, tendinopathy from overuse, and pain referred from the hip or lower back. Age-related cartilage wear, excess weight placing additional stress on joints, and poor posture can all contribute to knee discomfort without a clear injury event.
