Does Topamax Help Prevent Migraines?

Yes, Topamax (topiramate) is FDA-approved for migraine prevention and significantly reduces monthly migraine frequency. Clinical trials show that many patients have fewer migraines with topiramate, and some achieve at least a 50% reduction in migraine frequency. The medication affects several chemical and electrical signaling processes in the brain, although exactly how it prevents migraines is not fully understood. Treatment is usually started at a low dose and increased gradually under a clinician’s guidance. The dose depends on the patient, and side effects can include tingling, appetite or weight changes, and problems with memory or concentration. While effective, Topamax must be taken regularly, and it may take several weeks—sometimes after reaching an effective dose—to judge how well it is working. Do not stop topiramate suddenly without medical guidance; clinicians generally reduce the dose gradually because abrupt withdrawal can increase the risk of seizures, even in people taking it for migraine prevention.
