Why Do I Have Pain Under My Left Rib Cage?
Medically reviewed by Andre Stone | MD, University of Pittsburgh School of Medicine on August 25th, 2026.
Pain under the left rib cage can stem from several organs and structures in that area. The most common causes include muscle strain from physical activity, gas trapped in the colon, or inflammation of the stomach lining (gastritis). More serious conditions like an enlarged spleen, kidney stones, or heart problems can also cause left-sided rib pain. Digestive and musculoskeletal problems are among the possible causes of pain under the left rib cage. The pain’s location, timing, and triggers can offer clues, but pain quality alone cannot reliably identify the cause. Seek prompt medical advice if the pain persists, worsens with breathing, or occurs with fever, nausea, or vomiting. Call emergency services for severe or sudden pain, chest pressure, trouble breathing, fainting, sweating, or pain spreading to the arm, jaw, back, or shoulder.
