Does Spironolactone Lower Testosterone in Men?

Spironolactone blocks androgen receptors and may affect testosterone production, but its effect on testosterone levels in men varies. Some men may have little or no measurable decrease in circulating testosterone even though the medication still blocks some testosterone effects. Changes in testosterone levels vary with the dose, treatment duration, other medications, and the reason spironolactone is being used. There is no single expected percentage decrease or timeline for all men. Because of its antiandrogen effects, spironolactone can cause decreased libido, erectile dysfunction, and gynecomastia in some men. It is generally not a routine treatment for male pattern hair loss, and its use depends on the patient’s condition and treatment goals. Monitoring depends on why spironolactone is prescribed. Clinicians commonly check potassium and kidney function because the medication can cause dangerously high potassium, while testosterone testing may be appropriate when hormone levels are part of the treatment goal.
