Can Iron Deficiency Cause Shortness of Breath?
Medically reviewed by Linda Girgis | MD, Girgis Family Medicine on August 21st, 2026.
Yes, iron deficiency can cause shortness of breath, especially during physical activity or exertion. When your body lacks sufficient iron, it cannot produce enough healthy red blood cells to carry adequate oxygen throughout your body. This condition, called iron deficiency anemia, affects approximately 3% of men and 20% of women globally. As oxygen levels drop, your heart works harder to pump blood, and your lungs compensate by increasing breathing rate. You may notice breathlessness during activities that previously felt easy, like climbing stairs or walking. Other symptoms often accompany this shortness of breath, including fatigue, pale skin, cold hands and feet, and rapid heartbeat.
